What the visits look like

The nurse reviews the client's needs, care plan and any clinical instructions.
Care plan
Visit frequency and tasks are agreed with the care manager.
Regular visits
Visits happen at the agreed times, with notes sent after each one.
Review
Changes in the client's health are reported straight away, and the plan is reviewed regularly.
Funding and cost
We confirm the cost with you before anything is booked. Our quotes include travel.
| Support at Home and Home Care Packages | Funded through the client's provider. The care manager sends the referral and we invoice the provider using Support at Home service names. |
|---|---|
| NDIS | For plan-managed and self-managed participants. Funded from the support category in the participant's plan that covers nursing care. We are not a registered NDIS provider, so we can't support agency-managed plans. |
| Private | Clients and families can book directly. We'll give you the rate and the likely number of hours up front. |
Questions
Who can make a referral?
Care managers and coordinators, support coordinators, GPs and hospital teams, and clients or their families. For package clients we check with the provider before booking.
Do you provide registered or enrolled nurses?
Both, depending on the care needed. Complex clinical care and assessments are done by registered nurses.
Can you cover regional areas?
Often, yes. We recruit local nurses for each referral outside our established areas, so allow a little longer for the first visit.
Are your nurses registered and insured?
Yes. Every nurse is registered with AHPRA, holds $5 million professional indemnity and public liability insurance, has a current police check and works under our agreement.
Ready to refer?
Send the service, suburb and how soon it's needed. We'll come back with a date and cost.
