Please read the Associated Provider Agreement (National) before signing. The key points:

• You're an independent contractor and choose which referrals you accept.
• Contact the client the same day you accept a referral; initial assessment within 14 days.
• Service notes to office@ableagedcare.com after each visit; initial assessment reports within 5 business days of the appointment.
• Keep current: professional indemnity and public liability ($5m each), police check under 3 years, registration and screening checks. Tell us straight away if anything lapses.
• Give 14 days' notice before ending a client's care. Report incidents and complaints promptly; keep client information confidential.
• Invoices within 3 business days of month-end; paid within 30 days.
• 12-month term, then rolling monthly; either party can end it on 30 days' notice.

• You're an independent contractor and choose which jobs you accept.
• Contact the client the same day you accept a job; first visit (or quote inspection) within 14 days.
• Quoted work: written quote within 3 business days, and no work starts until we approve it.
• Licensed work only by licensed people; send any compliance certificate within 5 business days.
• Can't make a visit? Tell the client and us 24 hours ahead. 14 days' notice to stop a regular service.
• Keep current: public liability ($5m), police check under 3 years, licences and screening checks.
• Service notes after each visit (with before and after photos for maintenance jobs); invoices within 3 business days of month-end; paid within 30 days.
• No private arrangements, gifts or extra work direct with clients. Report incidents, hazards and complaints promptly.
• 12-month term, then rolling monthly; either party can end it on 30 days' notice.
