Support at Home and Home Care PackagesFunded through the client's provider. The care manager sends the referral and we invoice the provider using Support at Home service names.
NDISFor plan-managed and self-managed participants. Usually from Capacity Building: Improved Health and Wellbeing or Improved Daily Living. We are not a registered NDIS provider, so we can't support agency-managed plans.

Questions

Who can make a referral?

Care managers and coordinators, support coordinators, GPs and hospital teams, and clients or their families. For package clients we check with the provider before booking.

What's the difference from physiotherapy?

Physiotherapists assess and treat injuries and recovery; exercise physiologists focus on longer-term exercise programs for health conditions. Many clients see a physio first.

Are your clinicians registered and insured?

Yes. Every clinician is registered with AHPRA or their professional body, holds $5 million professional indemnity and public liability insurance, has a current police check and works under our agreement.

Ready to refer?

Send the service, suburb and how soon it's needed. We'll come back with a date and cost.
